Community Choice Waiver (CCW)

The Community Choice Waiver (CCW) helps eligible individuals access services and supports that promote independence, strengthen daily functioning, and make it possible to live in the community rather than in institutional settings when appropriate. These services are designed to help individuals maintain their quality of life while receiving support tailored to their needs, preferences, and personal goals.

We believe individuals deserve the opportunity to make meaningful choices about their lives and where they live. Through individualized support and connections to appropriate community-based services, we help individuals work toward greater independence, maintain daily routines, and participate more fully in their communities.
